Bridging the Gaps: Building a Community Information Exchange in Calgary

June 15 @ 12:00 pm 1:00 pm MDT

Join us for a fireside chat on building a Community Information Exchange (CIE) in Calgary with Geoff Zakaib and Richard Mugford. The social service sector in Calgary faces numerous challenges, particularly in providing the best support for people referred for complex and urgent care. Coordinated care planning and closed-loop referrals are effective solutions that require collaborative efforts among referring parties, service providers, and the client to ensure comprehensive and timely support. However, several barriers need to be addressed to make this approach successful.

This webinar will explore the potential impact of the CIE in Calgary, how it facilitates coordinated community care planning, and overcomes the barriers currently faced in the social service sector. The CIE, which is community-led, centers on shared language, resource databases, and integrated technology platforms, enabling proactive, holistic, and person-centered care. Through the CIE, social service providers, government agencies, emergency responders, and community partners can come together to address complex social issues, leading to greater equity in the provision of social services.

The webinar will discuss the work ahead in engaging with stakeholders to build a shared understanding of the CIE, developing necessary infrastructure to support the CIE, and ongoing monitoring and evaluation to assess its impact and identify areas for improvement. By bringing together a range of stakeholders and utilizing shared language, resource databases, and integrated technology platforms, the CIE has the potential to transform the social service sector in Calgary and create a more equitable and sustainable system for supporting individuals with complex needs.

Join us for this informative and engaging conversation on the potential of the CIE in Calgary and its impact on the social service sector.

Bring your questions, comments, and curiosity. We can’t wait to see you!
